CCTV Membership Form
If you want to become a member of the organization, fill out this form and send it in to us with your membership fee.
 
CCTV Policies and Proceedures
These are the rules of the organization.  Included are the rights of membership, liability, and minimal copyright information.
 
Program Submission Form
This form needs to be filled out and included with every program that is submitted for air.  (One form is needed per DVD, even if you have a continuing series.)
 
Talent Release Form
Every person who agrees to be on camera should fill out a Talent Release form.  This is protection for every producer who broadcasts a program on CCTV.
 
CORI form
Because we are located at Concord-Carlisle High School, all members of CCTV must complete a CORI form.
